International Host School and International Host School Coordinator Forms
These are forms for International Host Schools and International Host School Coordinators in the Latin America. A completed copy of each form must be sent to the ACSI Destinations office in Colorado Springs, CO, USA.
International Host School Application Form: To be filled out by the school’s administration. Provides important information about the school to ACSI Destinations.

Host Family Interview Form: To be used for in-home interviews with potential host families. All members of the family must be present for this interview.

Study Abroad Student Host School Enrollment Form: Confirms student’s enrollment and provides important information such as their host family placement, school tuition and fees, and homestay fees.

Individual background check authorization, release and acknowledgment: Authorizes ACSI to conduct a criminal background check on the individual who fills it out. It also releases ACSI and the host school from any liability associated with conducting a criminal background check on the individual. International Host school coordinators (IHSCs) and everyone else (besides host families) involved in the Destinations program must complete this form annually.
Background Check Form
International Host School Coordinator (IHSC) Contact Log: This form is for IHSCs to record their contact with the Destinations students, host families, and host school. At least once a month, IHSCs must check in with each student, connect with each host family, and check on each student’s progress at school. One log per student must be kept. At the end of every month IHSCs must send a copy of their contact logs to the Destinations office in Colorado Springs. Electronic submission is preferred.
